- Application for the supplementary pension allowance Please use this form if you want to apply for the supplementary pension allowance and you live within the EU, in Norway, Iceland, Liechtenstein or Schwitzerland.
- Life Certificate Once a year, pensioners abroad are requested by the Danish Pensions Agency to prove that they are still alive. Pen-sioners have to send both Life certificate and Declaration of marital status. For pensioners in Norway, Sweden, Germany, the Netherlands and Australia, however, there is normally no such Life certificate procedure.

- Declaration of income (certain countries outside the EU/EEA) Please use this form if there has been changes in your income and you live in Australia, Bosnia and Herzegovina, Canada, Chile, Israel, Croatia, Macedonia (FYROM), Morocco, New Zealand, Pakistan, Serbia-Montenegro, Turkey or USA.
- Declaraton of income (EU/EEA) Please use this form if you live within the EU/EEA or in Switzerland, and if there has been any changes in your income.
- EØS 4.1 - Application for document PD U1/ certificate E 301 DK or N 301 DK Application for document PD U1/certificate E 301 DK or N 301 DK (Form EØS 4.1) and Oplysninger fra arbejdsgivere V - Efter arbejde i Danmark (Form EØS 7) are used for application for document PD U1 / E 301. PD U1 / E 301 can be used for transferring Danish periods of employment and insurance, if you are to apply for unemployment benefits in another EEA country.

- Questionnaire for our investigation of your claim to an early Danish pension This questionnaire is a supplement to your application for an early Danish pension.
- Questionnaire for work within the EU after 1st of May 2010 Please use this questionnaire for an employed or self-employed person who takes up temporary work in one or more EU Member States from May 2010 or Switzerland from April 2012.
- Residence and tax liability Please use this certificate if Denmark and your country of residence have concluded an agreement on taxation of State pensions, and your Danish pension will be subject to taxation in your country of residence according to that agreement.
